But not every patient walking through that door is what they appear to be. Some of them are anomalies, dangerous creatures disguised as ordinary visitors. Learning to spot them is the key difference between surviving your shift and losing everything.
The game launched in early 2026 and quickly climbed into Roblox’s trending top 20, pulling in tens of thousands of concurrent players. The core loop is simple but deeply stressful: admit the normal patients, reject the anomalies, treat the sick, and keep your sanity from hitting zero. The horror here is not cheap jump scares. It is the slow, creeping dread of doubting whether the patient in front of you is actually real.
What Are Anomalies in Animal Hospital?
Anomalies are dangerous entities that disguise themselves as regular patients or visitors. According to Dr. Harlow in the game, anomalies hate having their photo taken, which is one of our most reliable tools against them. Not much is known in the game’s lore about where they come from or how animals turn into them. What we do know is that letting one through check-in puts both our patients and us at serious risk.

When we spot an anomaly at the check-in window, we close the shutters to reject them. This counts toward our shift score and earns us more cash for the shop. If we accidentally let one in, we have to deal with it using a taser or any weapon we have purchased. The game divides anomalies into three main categories based on how we detect them.
Anomalies You Can Spot With the Naked Eye
These are the most straightforward anomalies to catch. We do not need any equipment to see them, just attention. Here is what to look out for:
- Wide Eyes and Sharp Teeth: The patient has a deeply unsettling, distorted smile with sharp teeth and two glowing eyes with wrong proportions. Their voice will also sound low-pitched and distorted, which is a dead giveaway.

- Three Red Eyes: Any patient showing three glowing red eyes is always an anomaly. No exceptions. Close the shutters immediately.
- Hollow Face: This one has large hollow eyes with bags under them and a distinct frown. The emptiness in the face looks deeply wrong compared to that of normal patients.
- Unnatural Facial Appearance: There are two variants here. The first has large eyes with black pupils and a drawn-on smile. The second has small, wide eyes with realistic teeth. Both versions also carry a distorted voice.
- Twitching: The patient twitches in the arms, head, and neck. Sometimes the twitching is subtle enough that we can only catch it on the security camera, so do not rely solely on what we see at the window.
One thing we have learned from playing is that anomalies can carry more than one of these traits at once. So if something looks slightly off, check for a second warning sign before deciding.
Photo Anomalies
These are the sneaky ones. The patient looks completely normal at the window, but the photo we take of them tells a different story. This is exactly why Dr. Harlow tells us to use the camera. Do not skip the photo step just because the patient looks fine standing there.

When we take a photo, and it looks different from what we see in front of us, that is a red flag. The differences can show up as mismatched eyes, the wrong number of ears, an extra horn, or a missing horn. The photo may also display eerie, realistic facial features even when the patient appears normal in person. Static and grainy photos are another warning sign we should never ignore. The worst one is the cursed photo, which shows wide bloodshot eyes and a grin. If we pick that photo up and look at it, we lose 10 sanity points. The safest move is to leave it on the counter and just reject the patient.
Security Camera Anomalies
Some anomalies only reveal themselves on the security feed. These are the hardest to catch because we have to actively check the cameras while also managing patients and emergencies. Here is what we look for on the CCTV:
- Black Eyes: The patient’s face is covered by a black image of staring eyes on the camera feed.
- Unnatural Body: The patient appears heavily stretched and distorted on camera. Closing and reopening the feed makes their body shape change each time.
- Staring at the Camera: The patient stares directly at the CCTV at all times, even while walking. Normal patients do not do this.
- Void Black Patients: The patient appears completely black on camera, including their eyes. This is one of the easier camera anomalies to catch.

- Skinwalkers: Some skinwalkers only reveal themselves through the security feed. We recommend using the lobby camera for a side view of the patient, as the check-in camera angle can be deceiving.
- Different Ears: The patient’s ears look different on camera compared to what we see at the window. Pay close attention to ear shape and size.
Environmental Threats and Special Anomalies
Beyond the check-in window anomalies, Animal Hospital throws several enemy types at us during our shift. The Bed Monster is a long black rabbit creature with a disturbing smile. Looking directly at it drains our sanity, so the rule is simple: do not stare at it and keep moving. There is also a ceiling creature made of eyes and flesh that grows larger the more we look up. We keep our camera angle pointed downward to stay safe. Small black creatures that appear in groups steal our items and drain our sanity on contact. Running away sends them disappearing into black smoke.
There is also Barney, a special NPC who asks us to keep a suitcase safe. We strongly recommend rejecting his request. If we give him coffee and then take his photo, the result is deeply unsettling and confirms he is not what he seems. The game also features a Camera Figure that appears when checking a broken CCTV feed. If we do not exit quickly, we take a major sanity hit from a jump scare.
Tips for Surviving Longer
Managing sanity is just as important as treating patients. Drink coffee at the start of every shift and restock whenever we get a chance. Always take a patient’s photo before registering them, even if they look completely normal at first glance. Check the security cameras regularly throughout the shift, not just when something feels wrong. When in doubt, close the shutters. It is far better to reject a real patient by mistake than to let an anomaly through. Use the taser wisely and save it for situations where an anomaly has already gotten past us. The red line above a room number tells us a patient is inside, which helps us navigate quickly without wasting time searching.
Animal Hospital rewards careful observation over quick reactions. Every detail matters, from the way a patient stands to the sound of their voice. We keep our eyes open, our sanity high, and we never trust a smile that looks just a little too wide.
