Animal Hospital Journal Guide – Every Anomaly, NPC, and Achievement Explained

Animal Hospital added a new Journal feature during its Hazmat Team update. Players can find it in the lobby, just below the Classes menu. The Journal works like a personal codex. It tracks every anomaly and NPC we run into during our shifts, along with a list of achievements we can unlock along the way.

This feature gives players a clear reason to explore beyond the basic gameplay loop. Instead of just surviving shifts and moving on, we now have a reward system that pushes us to interact with every character and monster in the game. Each completed entry hands us a set number of Animal Coins, and those coins add up fast if we stay consistent.

The Journal only tracks encounters from the point of the update onward. So if we met certain NPCs or anomalies before the Journal launched, those meetings will not count. We need to trigger the events again to log them properly.

Breaking Down the Three Journal Categories

The Journal splits into three separate sections. Each one covers a different part of the Animal Hospital experience, and each rewards Animal Coins once completed.

Most players find the Anomalies and Achievements sections fairly straightforward. The NPC section takes more patience since some characters only appear during specific story branches or later shifts.

How to Complete Anomaly Entries

Anomaly entries usually require us to let a monster interact with us in a specific way, rather than simply avoiding it. This catches new players off guard at first, since most of our instincts push us to stay away from danger.

For example, we log the Ghost entry by letting it drain some of our Sanity. We complete the Stalker entry just by looking directly at it. The Bed Monster asks for either a Maple Syrup offering or getting grabbed by its hands. Slimebert, the slime anomaly introduced in the same update, only logs once it swallows us whole.

Most anomalies start appearing around Shift 10, so newer players will not see many of these events early on. Since several entries mean taking Sanity damage on purpose, it helps to stock up on Coffee, Mint Tea, or Chocolate Bars before we go looking for trouble. These items restore Sanity quickly and let us safely chase entries without risking a shift failure.

Tracking NPC Encounters the Right Way

NPC entries reward us simply for meeting characters and following their questlines to the end. Barney offers two different endings depending on whether we hand him over to Officer Duckman or hide him instead. Both choices unlock separate Journal entries, so completionists will need to experience the story twice across different runs.

Liz stands out as the toughest NPC entry to finish. She asks us to photograph specific patients by name, and one wrong photo ends her questline early. Once that happens, she will not return for the rest of that playthrough. We recommend writing down patient names the moment Liz mentions them, since the game gives no second warning.

Rathhew also hides a secret ending. We unlock it by removing the poster in Room 5 and opening the vault behind it. This entry rewards fewer coins than the others, but it adds a nice layer of discovery for players who like digging into hospital secrets.

Achievement Entries and Long-Term Progress

The Achievements section holds the most entries out of the three categories, and it takes the longest to fully clear. Many of these unlock naturally just from playing regular shifts. Treating patients, surviving shift milestones, and repairing security cameras all count toward specific goals.

A few achievements need more deliberate effort. Luring a Hider or Mimic into a Bed Monster’s attack range takes precise timing and usually works best with a coordinated team. Using the wrong treatment on a patient who turns out to be an anomaly also counts, which rewards players willing to take a calculated risk during check-ins.

Shift-based achievements offer the biggest coin payouts. Reaching Shift 100 alone hands us 250 Animal Coins, making long survival runs one of the most efficient ways to fund new classes.

Why the Journal Matters for Progression

The Journal changes how we approach Animal Hospital. Before this update, most players focused only on surviving each shift without dying. Now there is a clear incentive to interact with anomalies intentionally, track NPC storylines fully, and aim for specific in-game actions instead of playing it safe every time.

The coins earned from Journal entries also feed directly into unlocking better classes, which makes future shifts easier to manage. Players who treat the Journal as a side project early on will have a real coin advantage by the time they reach the harder late-game shifts.

For anyone chasing full completion, patience matters more than skill here. Several entries depend on timing, specific NPC dialogue choices, or waiting for certain shift numbers to unlock rare anomaly spawns. Working through the Journal steadily, shift after shift, remains the most reliable way to unlock every reward without unnecessary risk.
